Healthcare in Thailand for expats: hospitals, costs and insurance
How expats can choose Thai healthcare, understand public and private access, arrange payment and insurance, and prepare for an emergency.
Read the direct answer ↓
The short answer
Thailand has public and private healthcare, with extensive international-patient services in major cities. Access and payment depend on employment-linked social security, private insurance or self-payment. Choose routine and emergency facilities in advance, check direct-billing and pre-authorisation rules, and keep a medical summary, insurer contact and payment backup available.
Direct billing may still require pre-authorisation or a deposit.
Renewal, pre-existing conditions and co-payments matter as much as the headline limit.
Choose a routine and emergency facility
Identify a nearby clinic or hospital for ordinary care and a facility capable of handling serious emergencies. Confirm whether the required specialty, English support and insurer network are available at that location, not merely elsewhere under the same hospital brand.
Save 1669 for emergency medical services and keep the address in Thai, a map pin and nearby landmark. Give a trusted person access to the passport, policy, medication and allergy information needed during an admission.
Understand how treatment will be paid
Ask whether care will use self-payment, reimbursement, insurer direct billing or a Social Security Office registered facility. Direct billing may depend on the exact policy, network, treatment and pre-authorisation, and a hospital can still request a deposit while approval is pending.
Foreign employees may be covered through Thai social security depending on the employment arrangement and registration. Confirm the selected facility and benefits rather than assuming access to every public or private hospital.
- ✓Routine clinic and emergency hospital
- ✓Insurer and assistance numbers
- ✓Direct-billing and deposit process
- ✓Social-security facility if applicable
- ✓Emergency payment backup
Compare insurance beyond the annual limit
Review inpatient and outpatient cover, condition limits, deductibles, co-payments, pre-existing and related conditions, chronic care, cancer, mental health, maternity, evacuation, home-country care and renewal terms.
Visa evidence and medical suitability are different tests. A policy can produce an immigration certificate yet leave important treatment gaps, while another strong medical policy may not satisfy the exact certificate format of a visa route.
Keep medication and prevention organised
Carry a clinical summary and prescriptions using generic drug names. Check the Thai FDA traveller permit route before bringing controlled narcotic or psychotropic medicine and do not assume a prescription makes import automatic.
Review heat, air pollution, mosquito exposure, food safety and road risk for the region and lifestyle. Prevention and a good emergency plan can matter more often than a large hospital list.

Questions people ask next.
These answers are general. Use the dated official sources below and check the authority handling your exact case.
Is healthcare free for expats in Thailand?
No general free entitlement should be assumed. Access may be through employment-linked social security, private insurance or self-payment, each with its own conditions and facility rules.
Do expats need health insurance in Thailand?
It is a prudent part of most relocation plans, and some visa routes have specific insurance evidence rules. The policy should also fit the person's age, conditions, location and desired hospitals.
What is Thailand's medical emergency number?
The national emergency medical number is 1669. Keep the location in Thai and English and a map pin available because precise location information can be critical.
Will a Thai hospital bill my insurer directly?
Sometimes, when the hospital, insurer, policy and treatment are eligible and authorisation is obtained. Confirm the process and keep a payment backup because a deposit may still be requested.
